Understanding Bariatric-Metabolic Surgery: A Medical Solution for Intractable Obesity

Obesity occurs when there is excess fat accumulation in the body, resulting in body weight exceeding the normal range. One way to assess this is by using the Body Mass Index (BMI).
A person is considered obese if their BMI is ≥30 kg/m². However, for Asian populations, bariatric surgery may be considered for patients with a BMI above 35 kg/m², or a BMI above 30 kg/m² with high-risk comorbidities, such as type 2 diabetes mellitus, hypertension, or other metabolic disorders.
What is Bariatric-Metabolic Surgery?
Bariatric surgery is a surgical procedure aimed at promoting weight loss. Meanwhile, the term metabolic surgery refers to the benefits of surgery that extend beyond weight loss to correcting metabolic disorders, such as diabetes, high cholesterol, and high blood pressure.
Contrary to the common misconception that surgery simply aims to "shrink the stomach," this procedure works through several mechanisms, including:
- Limiting the amount of food consumed,
- Altering nutrient absorption during certain types of surgery,
- And affecting gastrointestinal hormones that play a role in hunger, satiety, and blood sugar control.
For this reason, many patients experience improvement in metabolic disease even before maximal weight loss occurs.

In addition to meeting BMI criteria, prospective patients must also undergo a comprehensive evaluation by a multidisciplinary team. Patients are expected to be motivated to make long-term lifestyle changes, willing to undergo routine lifelong check-ups, quit smoking, and be ready to maintain a diet and physical activity plan after surgery.

One of the most commonly performed procedures today is Laparoscopic Sleeve Gastrectomy (LSG) because it is relatively simple, provides good weight loss, has a low complication rate, and requires a shorter hospital stay compared to other techniques.
What About After Surgery?
The success of bariatric surgery is determined not only by the surgery itself, but also by lifestyle changes after the procedure.
Patients will undergo a gradual diet transition, starting with water, a high-protein liquid diet, pureed foods, and soft foods, until returning to a regular diet approximately two months after surgery. In addition, patients are advised to take vitamins as recommended by their doctor, exercise regularly, maintain adequate fluid intake, avoid foods high in sugar and fat, avoid carbonated beverages or alcohol, and quit smoking.
Obesity Treatment Must Be Comprehensive
Not all obese patients require surgery. Doctors will determine treatment options based on their health condition, BMI, comorbidities, and the patient's readiness to make lifestyle changes. Therefore, consulting with a doctor is an important step in determining the most appropriate treatment.
